Qualifications

• Bachelor's degree in counseling, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field.
• Two years of supervised clinical experience in a mental health setting if degree is in another field.
• CRSS certification through ICB is preferred/required in some cases.

Responsibilities

• Conduct and document mental health assessments and treatment plans.
• Provide case management including linking members to community resources and assisting with entitlements, housing, and services.
• Offer community support to achieve rehabilitative and recovery goals with illness self-management.
• Coordinate with medical providers to promote collaborative care and discuss treatment needs.
• Perform non-medical assessments to identify housing, transportation, employment, and community resources.
• Reach out via phone, email, and in person to reduce barriers to treatment.
• Facilitate life skills and therapeutic groups and provide counseling.
• Maintain caseload and monitor symptoms; assist with medication adherence and appointments.
• Assist clients in finding housing and establishing routines.
