Job summary

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, Business Administration, Communications, Public Health, or related field required.
• Bilingual proficiency in Spanish and English is highly desirable.
• Strong verbal and written communication; ability to manage multiple priorities and deadlines.

Responsibilities

• Conduct comprehensive participant assessments to develop individualized care plans addressing health, employment, and social needs.
• Coordinate referrals to workforce programs, healthcare providers, and community services.
• Facilitate workshops and counseling sessions focused on job readiness and career development.
• Maintain participant records and data entry in CalJOBS, CMS, and other systems.
• Provide up to 12 months of employment retention support and follow-up services.
