Job summary

Bay Area Community Services (BACS) is looking for a Care Coordinator in Sacramento to provide high-quality therapeutic interventions and case management services to individuals experiencing chronic homelessness. This community-based role requires direct, face-to-face service throughout Sacramento County.

The ideal candidate will have at least 4 years of experience in social services, specifically with the homeless population, along with a valid CA driver's license. Benefits include competitive hourly rates, health insurance, and generous PTO.

Qualifications

• At least 4 years of paid work experience in mental health/social services.
• Experience working with the homeless population is required.
• Valid California driver's license and personal vehicle is mandatory.

Responsibilities

• Provide therapeutic interventions and case management services.
• Develop service plans that are person-centered and client-focused.
• Document and maintain all data collection and reports.
