Job summary

Bay Area Community Services (BACS) is seeking a Care Coordinator/Case Manager in Fairfield, CA. This full-time, community-based role involves delivering direct, face-to-face services to chronically homeless individuals across Solano County, including housing navigation, crisis intervention, and family support.

Responsibilities include building service plans, engaging clients, and coordinating care with the outreach team, while traveling within the county and using a personal vehicle.

Qualifications

• High School Diploma required and 4 years paid work in mental health/social services.
• California driver’s license, personal vehicle, and proof of personal vehicle insurance required.
• Extensive local travel required, vehicle reimbursement is provided.

Responsibilities

• Develop and deliver a full complement of services including counseling, case management, and rehabilitation for individuals in the caseload.
• Utilize Evidence Based Practice to assure high quality results and impact.
• Assess risk and intervene accordingly based on symptoms, substance use and environment.
