Job summary

HCRS (Health Care & Rehabilitation Services of Southeastern Vermont) seeks a Community Case Manager for Adult Services. You will work directly with individuals to build functional skills, address barriers to care, and support follow-through on treatment plans and referrals.

The ideal candidate has an Associate's degree and two years of experience and/or a bachelor's degree, with strong collaboration, problem-solving, and interpersonal skills.

• Experience working in the mental health field.

Job description

The pay for this position is $44,850-$58,909 annually.

$500 sign-on bonus

The Community Case Manager works directly with individuals receiving services to build functional skills, address barriers to care and social and mental wellbeing, and support follow-through on treatment plans and referrals.
