Job summary

HCRS (Health Care & Rehabilitation Services of Southeastern Vermont) is seeking a Care Navigator Case Manager for children's services. In this role, you will lead service planning and care coordination for families receiving mental health services. Responsibilities include managing referrals, providing primary care screenings, and liaising with community partners. Candidates should possess an Associate’s Degree or Bachelor’s Degree, along with the ability to establish supportive relationships and experience in the mental health field. A comprehensive benefits package and a competitive salary range of $44,850-$58,908.50 annually are offered.

Qualifications

The pay for this position is $44,850-$58,908.50 annually.

Job Responsibilities

Sometimes it takes only one person to make a difference in a life. As the Care Navigator Case Manager for children’s services, you could be that person. The Care Navigator Case Manager leads service planning and care coordination navigation for children and families receiving mental health services, including managing referrals, providing primary care screening and monitoring, and serving as a primary liaison with community partners.
