Qualifications

• Must have a BSW degree from an accredited program.
• Licensed Social Worker in KY and IN (LSW/SW).
• 0–3 years of experience.
• Ability to coordinate community resources and education.

Responsibilities

• Comply with all applicable laws and regulations.
• Provide patient-centered evaluation and care planning, resource coordination, and health education during home visits.
• Perform face-to-face visits in PCP offices, community settings or home environments.
• Document visits and interventions in the chart accurately and timely.
• Coordinate with physicians, nurses, and care teams to meet patient social needs.
