Job summary

St. Elizabeth's Health at Home is seeking a Medical Social Worker for a PRN position. The role involves coordinating care plans, crisis intervention, and addressing social and economic factors affecting patient outcomes.

The candidate will educate patients and caregivers, refer to community resources, and collaborate with physicians and other caregivers while ensuring timely documentation and compliance with regulatory requirements.

Qualifications

• Master’s degree from CSWE-accredited program.
• Currently licensed to practice in the state served.
• One year of social work experience in a health-care setting.
• Current CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements

Responsibilities

• Establishing a care plan in accordance with the physician’s plan of treatment.
• Providing crisis intervention as required.
• Assisting the physician and other caregivers in understanding social, emotional and economic factors affecting medical improvement.
• Providing patient care education to patients and caregivers.
• Assisting in making community referrals for public assistance, housing, transportation, and health resources
• Working with caregivers to resolve problems in the patient-caregiver relationship
• Completing and submitting all required documentation accurately and timely

Job Summary and Responsibilities

• Establishing a care plan in accordance with the physician’s plan of treatment.
• Providing crisis intervention as required.
• Assisting the physician and other caregivers in understanding the significant social, emotional and economic factors influencing the patient’s ability to improve medically.
• Providing patient care education to patients and caregivers.
• Assisting in making appropriate community referrals to gain access to additional resources as needed (i.e. applications for public assistance, housing problems, additional health care resources, transportation)
• Working with the patient’s caregivers to resolve problems interfering with an optimum caring relationship between the patient and caregiver.
• Completing and submitting all required documentation in an accurate and timely manner.

Job Requirements

**This is a PRN Position**

Qualifications:

• Master’s degree from an accredited school approved by the Council on Social Work Education. In Indiana, must also be certified under IC25-23.6-4-3.
• Currently licensed to practice in the state serviced.
• One year social work experience in a health care setting.
• Current CPR/BLS certification through American Heart Association or American Red Cross and compliance with state requirements
