Job summary

LifePath, Inc. is seeking a Community Options Resource Specialist in Greenfield, MA.

This full-time role focuses on building supportive relationships with older adults and people with disabilities, creating individualized service plans, and advocating for access to resources in Franklin County and North Quabbin. The position emphasizes coordination with care partners, documentation, and collaboration with a range of providers.

Qualifications

• Bachelor’s Degree preferred and 2-3 years of health or human services experience required.

Responsibilities

• Conduct virtual and in-person comprehensive, person-centered assessments with consumers and care partners.
• Develop and update a service plan with consumer and support network.
• Make referrals to community-based agencies and governmental organizations.
• Collaborate with providers to coordinate service plans and meetings.
• Maintain consumer records per regulations.
• Assist families and caregivers in problem-solving and attend meetings.
