Job summary

Qualifications

• Bachelor’s Degree preferred; 2–3 years in health or human services required.
• License in Social Work is a plus.
• Willingness to work with older adults and individuals with disabilities.
• Strong documentation skills and experience with EHR or care management platforms and Microsoft Office.

Responsibilities

• Conduct in-person assessments with clients, care partners, and health professionals.
• Develop and update service plans with consumer and supports.
• Authorize internally funded services.
• Refer to community resources and governmental organizations.
• Coordinate with providers to ensure service plan alignment.
• Maintain consumer records per regulations.
• Assist families and facilitate team meetings and conferences.
• Stay informed about services for older adults and disabilities.
• Advocate for consumers to obtain needed services.
