LifePath Inc. is seeking a Community Options Resource Specialist to support older adults and people with disabilities in the Greenfield area. You will assess needs, develop service plans, and refer clients to community resources while advocating for access to essential services.

The role emphasizes collaboration with care partners, documentation in EHR systems, and ongoing communication with an interdisciplinary team to foster independence and well-being.

Qualifikationen

• Bachelor’s degree preferred and 2-3 years of experience in health or human services.
• License in Social Work is a plus.

• Develop supportive relationships with older adults and people with disabilities.
• Disseminate information and make referrals; create service plans.
• Advocate for consumers and coordinate with providers.
• Conduct assessments with consumers, care partners, and health-care professionals.
• Maintain consumer records per regulations and participate in team meetings.
