Job summary

Responsibilities include outreach to PCPs, completing paperwork, and coordinating with therapy centers while tracking visits and outcomes. Travel in-state may be required.

Qualifications

• Current State License as a Licensed Practical Nurse (LPN).
• Valid driver’s license and automobile insurance liability.
• Graduate of a state approved accredited nursing school.

Responsibilities

• Screens referrals and coordinates admission process.
• Coordinates services with providers and handles billing reconciliation.
• Outreach to community referral sources.
• Participates in team meetings and presents new admits and discharges.
• Completes reassessments, reviews medications, and arranges transportation as needed.
