Job summary

Grace at Home is seeking an outreach-focused Care Guide to bridge the human and medical sides of care in our community-based program. You’ll listen, connect, and elevate stories that matter, helping Family Members feel supported on their journey.

Join us to engage high-need communities, address barriers to care, coordinate with nurses and social workers, and document interactions to improve care delivery. This role emphasizes empathy, cultural awareness, and reliable in-person outreach.

Qualifications

• Associate degree or combination of education and experience required; B.S. or B.A. preferred.
• 1–3 years of experience in community-based outreach or health education.
• Valid driver's license and reliable transportation required.
• Background check and training required prior to field deployment.
• Bilingual or multilingual skills (e.g., Spanish, Haitian Creole, Russian) a plus.
• EMT experience preferred.

Responsibilities

• Engage Family Members in their homes and communities with culturally aligned communication.
• Build trust through consistent, empathetic outreach and collaboration with care teams.
• Address barriers to care such as food, housing, and transportation via referrals.
• Assist with paperwork for social services and basic support needs.
• Support in-home technology setup to enable virtual visits and connect with the care team.
• Provide warm handoffs and referrals and follow up to ensure needs are met.
• Document interactions and track engagement levels in CINQIQ.
• Represent CINQCARE at community events with a local network of trusted contacts.
