Job summary

Praesum Healthcare in Texas is seeking a Community Care Coordinator to develop and grow referral networks with primary care physicians, hospitals, therapists and other referral sources. You will meet weekly activity targets for outreach, expand referrals across Praesum programs, and collaborate with senior management to maximize revenue while supporting marketing efforts.

Responsibilities

• Develop and grow professional relationships with physicians, hospitals, therapists and other referral sources via door‑to‑door visits, calls, in‑person meetings, presentations and social media.
• Consistently meet weekly and monthly activity targets for outreach and referrals.
• Contribute referrals from personal accounts across Praesum Healthcare programs and locations.
• Understand geographic areas to optimize referral relationships for detox and counseling programs.
• Assist in marketing/PR materials to build relationships locally and with the company.
• Participate in seminars and campaigns to obtain prospective referral resources.
• Work with senior management to maximize revenue by increasing referrals for the centers’ services.
• Support marketing during key meetings with executives, including travel.
• Record daily activity through CRM software.

Praesum Healthcare, founded in 2004, has grown to include over thirty centers across six states. With a proven track record in mental health and substance recovery, Praesum is accredited by the Joint Commission and holds required licenses and certifications in each state of operation. Sunrise Detox Centre provides medically supervised detox programs for alcohol, opiates, and prescription drug use, with ten locations in New Jersey, Georgia, Florida, New York and Massachusetts. Additional outpatient and residential services are offered through the Counseling Center and Evolve Recovery Centers across the region.

Position Title

Community Care Coordinator

Responsibilities

• Develop and grow professional relationships with primary care physicians, hospitals, therapists and other referral sources through door‑to‑door visits, telephone outreach, in‑person meetings, presentations, professional organizations and social media.
• Consistently meet weekly and monthly activity expectations for door‑to‑door check‑ins with potential referral partners.
• Contribute monthly referrals from personal accounts across all of Praesum Healthcare’s Behavioral Health programs and locations, focusing on the local market.
• Understand geographic areas to create professional referral relationships that best utilize detox and counseling treatment programs.
• Assist in the development of marketing and PR materials for building and maintaining relationships within the geographic area and/or company.
• Contribute to the development of or participate in seminars and company campaigns to obtain prospective referral resources.
• Work closely with senior management of local facilities to help obtain and maintain optimal revenue by maximizing referrals for the center’s treatment programs and services.
• Assist and support marketing department personnel at key meetings with executive‑level audiences, including daily local travel and occasional overnight travel, which may involve nights and weekends.
• Record daily activity through CRM software.
