In some areas, this may include pre-admission to post discharge coordination of care.**Position Summary**The Clinical Case Manager monitors the patient care process and physiological needs; documents, communicates, and collects data to evaluate and improve timeliness and appropriateness of care. The Clinical Case Manager collaborates with physicians, nurses and other health care workers to provide comprehensive case management assessment, treatment, and follow-up evaluation for patients and to communicate plan of care. The Clinical Case Manager maintains an awareness of state and national health care trends and proactively responds with initiatives to enhance provision of care and ensure efficient resource utilization. **Minimum Qualifications**RN license and Bachelor of Science in Nursing required. 2 years clinical experience applicable to the patient population, home care, hospice, community agency, and/or payer relations required. Prior case management experience strongly preferred. This role must understand the health care continuum and have the ability to solve complex problems and possess strong communication and interpersonal skills.
