Job summary

Grace Medical Center, part of LifeBridge Health, seeks a Community Care Coordinator to promote patient health through face-to-face, phone, and email outreach within the IDT framework.

You will assess needs, connect patients with community resources, and ensure timely access to services while respecting patient and family preferences.

Preferred candidates have 1-3 years of relevant care-management experience, EMR competency, and may hold LPN or CMA credentials.

Qualifications

• Minimum 1 year of experience in care management or related field.
• Education preferred: High School / GED.
• LPN or CMA credentials with relevant licensure preferred.

• Promotes the health and welfare of assigned patients through face-to-face, phone, and email outreach.
• Collaborates with the Interdisciplinary Team (IDT) to identify patient needs and address them promptly.
• Advocates for patients, linking to community resources and ensuring timely access to services.

Job description

Summary

Promotes the health and welfare of assigned patients through face to face and/or phone outreach and e-mail communications. The Community Care Coordinator is a member of Interdisciplinary Team (IDT) caring for the patient in ensuring the patient's individual needs are identified and addressed in a timely manner, act as patient advocate to address primary physical and social needs including assessing and linking community resources available to the patient, as well as ensuring patients assigned have timely access to services they need while respecting the rights and wishes of the patient and family.

Experience

Required: 1-3 years

Experience Note:

• Preferably with experience working with care managers from acute care setting or health insurance and/or other payer entities
• Competency in electronic medical records desirable
• Bi-lingual preferable (market specific)

Education

Preferred: High School / GED

Licensures & Certifications

Preferred:

• Licensed Practical Nurse - Current Maryland license or eligibility to obtain Maryland license
• Medical Assistant Certification

Credential Note:

• Licensed Practice Nurse or Certified Medical Assistant or trained Patient Care Assistant with 2-3 years acute care and/or ambulatory practice experience

Pay Range

$20.00 to $26.00 Based On Experience
