Job summary

Spring Health seeks a Community Care Advocate to connect employees with community and company resources addressing social determinants of health and employee needs. This onsite role requires schedule Mon-Fri in Chicago ZIP 60640/60608, with referrals, crisis support, and navigation of mental health benefits.

You’ll refer with internal care teams, document outcomes, and promote the service to members while using tech-enabled tools. Fluent Spanish and a related bachelor’s degree are required.

• Experience working with lower income populations.
• Tech-savvy: comfortable using various computer platforms.
• Experience assisting individuals through burnout and managing work-life balance.

Responsibilities

• Identify and make referrals for work-life, financial & legal, and basic needs support.
• Link employees to local and national resources via one-on-one consultation and referral lists.
• Work with internal care teams on emergency funding and resource referrals.
• Offer consultation related to navigating mental health resources and workplace benefits.
• Assist in linking employees to mental health treatment options and provide support throughout applications and follow-ups.
• Document metrics/outcomes including demographics, referrals, and success of referrals.
• Represent Spring Health and reinforce program parameters and goals.
• Communicate with local points of contact and escalate needs to Spring Health partners.
• Submit reporting documentation of all client and organizational activities.
• Develop relationships with key customer contacts and attend customer meetings as requested.
• Promote the community care advocate service offering to employees.
