Job summary

You will identify referrals for work-life, financial, legal, housing, and other supports, link employees to resources, and follow through with applications while collaborating with internal care teams to ensure timely assistance.

Qualifications

• Ability to balance multiple stakeholders.
• Excellent listening, written and verbal communication skills.

Responsibilities

• Provide one-on-one referrals for work-life, financial & legal, and basic needs resources.
• Link employees to local and national resources via consultations and follow-ups.
• Collaborate with internal care teams on emergency funding referrals.
• Advise on navigating mental health resources and workplace benefits.
• Document and track referral outcomes and metrics (referrals placed, outcomes).
• Promote the community care advocate service to employees.
