Job summary

Chimes is seeking a Case Manager to provide community-based behavioral health services in the Newark area. You will carry a caseload, assess needs, and develop strength-based service plans to promote recovery and stable living in the community.

The role involves coordinating with families, payers, and service providers, plus maintaining timely documentation. A 24/7 on-call rotation is required. This position supports individuals across Pennsylvania, New Jersey, Delaware, and Maryland.

Qualifications

• Bachelor’s degree with major coursework in sociology, social work, psychology or related fields.
• Registered nurse licensure required.
• High school diploma plus 12 college credits in social sciences and 2 years in human services.
• High school diploma plus 5 years of mental health direct care experience.

Responsibilities

• Carry a caseload of up to 30 clients.
• Perform case management duties for consumers in Behavioral Health Services.
• Develop individualized, strength-based service plans that are measurable and lead toward targeted outcomes.
• Assist consumers in obtaining basic needs such as housing, food, healthcare, employment, and socialization.
• Provide crisis assessment and crisis intervention when necessary.
• Maintain professional relations with consumers, families, payers, and community staff.
• Act as a single point of contact for health and social service linkages.
• Submit accurate payroll and billing documentation.
• Be available for an on-call rotation 24/7.
