Job summary

Lutheran Community Services NW is seeking a Mental Health Clinician for the SWIFT program to deliver intensive crisis intervention and supportive services to youth and families. You will coordinate care, adapt therapy sessions to needs, and collaborate with a coordinated care team across multiple sites.

Responsibilities include maintaining a youth caseload, ensuring safe transition planning, crisis response, and warm handoffs to ongoing providers.

Qualifications

• 1 year of related experience providing behavioral health services to youth and families.
• Active Washington State DOH behavioral health credential required or must apply for Agency Affiliated Counselor credential within 30 days of hire.

Responsibilities

• Maintain a caseload of up to 20 youth and families with services transitioning within 30 days and continuing up to 60–90 days when appropriate.
• Partner with the Care Coordinator to assess needs and plan for safety.
• Provide flexible therapy sessions based on identified needs.
• Deliver rapid, community-based crisis intervention in outpatient, referral, and SWIFT programs.
• Use crisis-intervention techniques to assess risks and support stabilization.
• Update individualized support plans and coordinate aftercare with other providers.
