Job summary

Walker Therapeutic & Educational Programs seeks a Therapeutic Mentor/TT&S to provide community-based therapeutic support to children, adolescents, and families. The role emphasizes skills coaching, mentoring, and family support within a coordinated care approach.

The position collaborates with a clinical treatment team to implement goals and strategies from each youth's therapeutic plan, aiming to strengthen stabilization and well-being in home and community settings.

Qualifications

• Bachelor's degree in social work, counseling, psychology, or related field.
• Must be able to obtain a certification as a community health worker within 2 years of employment.
• Preferred: Certified Community Health Worker.

Responsibilities

• Provide community-based support, skills coaching, and mentoring to youth and families in home, school, and community settings.
• Implement therapeutic goals and safety strategies in collaboration with clinical supervisors and treatment teams.
• Support youth in developing coping skills, emotional regulation, communication, and problem-solving abilities.
• Assist caregivers in strengthening routines, behavior management, communication, and overall family stability.
• Facilitate coordination with providers, natural supports, and family members to ensure consistent, integrated care.
• Maintain accurate and timely documentation, attend required meetings and supervision, and participate in ongoing training.
• Help connect families to resources and supports that promote safety, stability, and long-term success.
• Additional duties as assigned
