Job summary

Ocean State Job Lot is seeking Direct Care Service Providers to support adults and youths with developmental or intellectual disabilities. The role involves coordination of services, documentation, and person-centered planning to help individuals access medical, social, educational, and vocational supports within the community.

Applicants should have a bachelor’s degree in a human services field and be able to collaborate with families, substitute decision makers, and multiple agencies.

Qualifications

• Bachelor's degree in a human services field required.
• Experience working with individuals with developmental or intellectual disabilities preferred.
• Valid Driver’s License with a safe driving record required.
• Ability to coordinate with multiple providers and agencies.
• Strong record keeping and data entry skills.

Responsibilities

• Liaison with state and community program partners to coordinate services for individuals served.
• Maintain accurate documentation and records for service delivery and reimbursement.
• Assess eligibility and service needs of individuals served.
• Monitor progress and update ISP and care plans on a quarterly basis.
• Refer individuals to residential, employment, and other waiver services as needed.
• Lead planning and ISP development with the individual, family, and providers.
• Ensure crisis response and after-hours availability when needed.
