Job summary

Telecare's FPATH #269 is seeking a Washington State Substance Use Disorder Professional (SUDP) to work in the community with adults living with serious mental illness and co-occurring substance use challenges.

You will provide substance use counseling, case management, and recovery support in home and community settings, using harm reduction and recovery-focused approaches. This role is fully in-person with no remote option.

Qualifications

• High School Diploma or GED or equivalent is required.
• Current Washington State SUDP credential is required.
• At least six months of experience in addictions/substance use setting is required.
• Ability and willingness to provide services throughout the community.

Responsibilities

• Provide substance use counseling, education, recovery support, and case management.
• Build relationships with individuals who may have struggled to engage in traditional treatment.
• Use harm reduction, motivational, and recovery-oriented approaches to support change.
• Help members identify their own goals related to substance use, mental health, housing, relationships, health, and independence.
• Meet members in their homes and throughout the community.
• Connect individuals with housing, healthcare, benefits, treatment, and community resources.
• Help members strengthen daily living and community integration skills.
• Collaborate with a multidisciplinary behavioral health team.
• Support individuals experiencing complex behavioral health and social needs.
• Advocate for members while helping them strengthen their own self-advocacy skills.
