Job summary

Telecare's FPATH #269 in Washington is seeking a certified Substance Use Disorder Professional (SUDP) Case Manager to work in the community with adults living with serious mental illness and co-occurring substance use challenges.

This is a fully in-person, community-based role with no remote option. You will meet people in their homes and in community settings, helping them address housing, healthcare, daily living needs, and recovery goals with harm-reduction and recovery-focused approaches.

Qualifications

• High School Diploma or GED.
• Current Washington State SUDP credential.
• At least six months of experience in addictions/substance use.
• Ability to provide services throughout the community.

Responsibilities

• Provide substance use counseling, education, recovery support, and case management
• Build relationships with individuals who may have struggled to engage in traditional treatment
• Use harm reduction, motivational, and recovery-oriented approaches to support change
• Help members identify goals related to substance use, mental health, housing, relationships, health, and independence
• Meet members in their homes and throughout the community
• Connect individuals with housing, healthcare, benefits, treatment, and community resources
• Help members strengthen daily living and community integration skills
• Collaborate with a multidisciplinary behavioral health team
• Support individuals experiencing complex behavioral health and social needs
• Advocate for members while helping them strengthen their own self-advocacy skills
