Job summary

Apalachee Center, Inc. is seeking a Substance Abuse Specialist within our FACT team in North Florida. You will deliver intensive community-based substance abuse services, conduct assessments, and help clients build coping skills to promote recovery and reduce relapse.

The role includes crisis intervention, collaboration with ITT members, and after-hours on-call coverage using Avatar for documentation and care planning.

Qualifications

• Bachelor’s degree in psychology, social work, counseling, or a related human services field.
• Master’s degree in psychology, social work, counseling, or a related field preferred.
• Two years of related professional experience in substance use disorders.
• Assertive community treatment experience with PACT model is preferred.

Responsibilities

• Conducts comprehensive mental health assessments and ongoing assessments of clients' status and response to treatment.
• Provides individual, group and family basis substance abuse services to promote recovery and prevent relapse.
• Coordinate with the ITT and FACT team to review progress and employment goals within treatment plans.
• Deliver crisis intervention and after-hours on-call support as scheduled.
• Document progress notes and CSP updates in Avatar within established guidelines.
