Job summary

Avento Health in East Moline, Illinois is seeking a Registered Nurse (RN) for a full-time community-based care role. The RN will provide clinical nursing, medication oversight, care coordination, staff education, and health advocacy outside hospital settings.

You will work with adults with intellectual and developmental disabilities, collaborate with physicians, pharmacies, families, and care teams, and lead staff training on medication administration and health-support needs.

Qualifications

• Valid Illinois RN license required.
• Must possess or be able to obtain Illinois RN-Trainer certificate.
• Driver’s license and automobile insurance required.
• Strong nursing assessment, clinical judgment, communication, interpersonal and documentation skills.
• Ability to travel locally as required.
• Ability to obtain first aid, CPR, medication-administration competencies.

Responsibilities

• Provide nursing care and health support in accordance with professional standards and regulations.
• Assess illness, injury, or changes and provide nursing guidance.
• Consult with direct-care staff on medical concerns; participate in on-call as needed.
• Train and oversee direct-care staff in medication administration and health-support needs.
• Educate on medical conditions, seizure disorders, diets, choking risks, equipment, and health needs.
• Coordinate medical, lab, health, and dental appointments and follow-ups.
• Administer medications per physician orders and review medication records.
• Coordinate with pharmacies and healthcare providers on medication orders.
• Document nursing assessments and health-risk records accurately and timely.
• Communicate condition changes to care team, families, or guardians.
