You will promote safety, independence, and meaningful relationships while advocating for self-determination.

Responsibilities include creating person-centered plans, assisting with transportation, documenting activities, and fostering inclusion.

Formación

• High School Diploma, GED or equivalent required.
• Valid Driver’s License and acceptable driving record.
• Reliability and openness to new opportunities.

Responsabilidades

• Assist individuals with daily living activities and personal goal attainment.
• Support transportation and community resource access.
• Develop and implement individualized service plans.
• Maintain accurate documentation in a timely manner.
