• Ability to work independently with minimal supervision.

Aufgaben

• Provide PRP services to assigned clients and document encounters the same day.
• Attend weekly staff meetings and coordinate with the care team.
• Collaborate with therapists and other care providers to follow IRP plans.
• Assist clients with accessing entitlements and benefits.
• Monitor progress and support independence and wellness goals.
• Deliver blended services onsite and in home/community settings.
