Job summary

Sertoma Star Services is seeking a Personal Support Worker (PSW) to provide community-based support to individuals with intellectual and developmental disabilities. The role focuses on person-centered care and helping consumers live independently in home and community settings.

Responsibilities include communicating with consumers, fostering independence, and supporting participation in daily activities. Requires compassionate, respectful care, and the ability to document progress and coordinate

Qualifications

• Excellent verbal and written communication skills.
• Ability to work effectively with coworkers and consumers.
• Knowledge of working with individuals with disabilities.
• Ability to work independently and complete assignments per instructions and routines.

Responsibilities

• Communicates with consumers using their preferred communication method to determine likes and needs.
• Treats consumers with dignity and respect while providing needed support.
• Help consumers achieve outcomes documented in their Person-Centered Individual Service Plan.
• Provide training and assist consumers by doing tasks with them, not for them.
• Advocate for consumers and help them advocate for themselves as requested.
• Support consumers to participate fully in their community.
• Promote community inclusion by developing natural supports.
• Encourage learning and decision-making by offering choices.
• Assess risks and help consumers make informed decisions when trying new experiences.
