Sertoma Star Services in South Holland, IL is seeking a Direct Support Professional to join our Community Day Services team. You will work alongside individuals with intellectual/developmental disabilities, providing hands-on support, training, and encouragement to build skills, independence, and meaningful participation in daily life.

This on-site role emphasizes person-centered planning, documentation in HSMS, transportation support, and teamwork with families and staff to ensure high-quality

Qualifikationen

• High School Diploma or GED required.
• Must maintain a valid driver’s license and meet driving requirements when transportation is needed.
• Obtain Pace certification within 3 months of hire; Maintain while working in CDS program.
• Good verbal and written communication skills.
• Ability to work independently and with coworkers/consumers.

Aufgaben

• Communicates with consumers using their preferred communication form to determine likes and needs.
• Treats consumers with dignity and respect while promoting independence.
• Supports consumers to achieve outcomes outlined in Personal Plan/Implementation Strategy.
• Provides training by completing tasks with the consumer, not for them.
• Assists consumers in learning job skills on the production line.
• Advocates for consumers and helps them self-advocate.
• Supports community participation and natural supports development.
• Documents progress notes, incidents, and attendance in HSMS timely.
