Community Options, Inc. is seeking a Licensed Practical Nurse in Pittsburgh, PA to deliver care in a community setting. You will assess, plan, implement, and evaluate nursing care for individuals with disabilities, under supervision of the RN and healthcare team.

Starting pay is $32.00 per hour. Responsibilities include health assessments, care planning, mandatory documentation, and training staff on medical needs.

Qualificações

• Bachelor’s degree or equivalent experience in nursing field.
• Minimum of one year of experience providing nursing services to individuals with disabilities.
• Valid RN license.
• Valid driver’s license with a satisfactory driving record.
• Ability to calculate figures related to medication dosages.
• Understanding and commitment to community-based support for persons with disabilities.
• Ability to exercise good judgment and remain calm in crisis situations.
• Excellent verbal and written communication skills.

Responsabilidades

• Provide treatment and health care procedures ordered by a physician and required by standards of practice.
• Ensure individuals attend medical appointments per their ISP.
• Advise supervisor of all concerns related to nursing services.
• Create and distribute medical/health reports detailing health goals and physician recommendations.
• Conduct trainings including medication administration and delegation.
• Develop and update ISPs/PCPs with an interdisciplinary team.
• Train employees on the medical needs of assigned individuals.
• Monitor health and refer for medical services as needed.
• Monitor medication use via administration logs.
• Ensure health supplies are available and replenished.
• Complete documentation and billable records accurately and timely.
• Build a support network with community members, businesses, and families.
• Participate in community, social, and recreational activities.
