Job summary

Community Options, Inc. is hiring a Licensed Practical Nurse/RN in Pittsburgh, PA. The L.P.N./RN delivers care in a community setting using the nursing process of assessment, planning, intervention, implementation and evaluation.

The role involves interaction with individuals, families, and health team members while upholding nursing standards. Starting pay is $32.00-$36.00 per hour. Requirements include a nursing license, 1+ year experience with individuals with disabilities, and a valid

Qualifications

Bachelor’s degree or equivalent nursing experience.
At least 1 year of nursing services to individuals with disabilities.
Valid nursing license.
Valid driver’s license with satisfactory driving record.
Ability to calculate medication dosages.
Ability to establish supportive relationships with individuals.

Responsibilities

Provide treatment and health care procedures ordered by a physician and required by standards of practice or law.
Ensure individuals attend medical appointments per ISP.
Advise supervisor of concerns related to nursing services.
Develop and update ISPs/PCPs with interdisciplinary team.
Train employees on medical needs of assigned individuals.
Monitor health conditions and refer to medical services as needed.
Monitor medication administration logs.
Complete program documentation and billable records accurately.
Build a support network with community members, businesses, and families.
Cooperate with license and department staff during inspections.
