Community Options, Inc. in Mechanicsburg, PA seeks a Licensed Practical Nurse to deliver care in a community setting. You will perform nursing assessments, plan interventions, and coordinate with health team members to support ISP/PCP goals.

Starting pay is up to $36.00 per hour. A nursing license is required, plus a bachelor’s degree or equivalent nursing experience and a valid driver’s license. Employment is contingent upon background checks.

• Ability to effectively present facts and recommendations
• Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing

• Ensure individuals attend medical appointments per their ISP
• Advise supervisor of all concerns and issues related to the provision of nursing services

• Train employees on the medical needs of assigned individuals
• Monitor health and medical conditions of individuals and refer for medical services as needed
• Monitor the use of medications through medication administration logs
• Ensure program documentation and billable records are completed accurately and timely
• Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
• Additional tasks and responsibilities may be assigned
• Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation

Community Options, Inc. is a national non‑profit agency providing services to individuals with disabilities in 11 states. We are now hiring a skilled Licensed Practical Nurse in Mechanicsburg, PA. The L.P.N. delivers care to individuals in a community setting utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. This person will interact with individuals, significant others, and health team members while maintaining the standards of professional nursing.
