Reach for Resources, Inc. in Minnesota is seeking Independent Living Skills Coaches to teach life skills, build independence, and support adults with disabilities and mental health conditions.

The role involves traveling to homes and community settings in Hennepin County, with occasional visits to the Edina office and no overnight shifts. The position offers a supportive environment, mileage reimbursement, a comprehensive benefits package, and a pay rate of $19 per hour plus travel

Qualifications

• Knowledge of disability services, mental health, or related field.
• Strong communication and time-management skills.
• Ability to operate a motor vehicle and travel as part of the role.
• Post-high school education or relevant experience; substitute for formal education if applicable.

Responsabilités

• Teach practical life skills and independence, through individualized teaching and coaching.
• Provide community-based support in homes and community settings.
• Develop, implement, and periodically evaluate Community Living Plans with individuals, families and teams.
