Job summary

Placement Partners MN, Inc. is seeking an Employment Support Specialist to help adults with disabilities achieve independence through meaningful employment and community involvement.

You’ll meet clients in the community, at workplaces, and at homes to provide job coaching, career exploration, and life-skills training. You will collaborate with employers, families and interdisciplinary teams to support employment success, document progress, and coordinate services that empower consumers to build

Qualifications

• Must be at least 18 years old and have a valid driver's license with a reliable vehicle.
• Proof of up-to-date auto insurance required.
• Experience working with individuals with developmental disabilities in social services is not required but preferred.

Responsibilities

• Assist individuals with disabilities in exploring career interests and securing competitive community employment.
• Provide resume creation, employer outreach, application assistance, interview preparation, and job offer guidance.
• Deliver on-the-job coaching and mentoring including onboarding, transportation, and barrier resolution.
• Support long-term employment retention through training, advocacy, problem solving and coordination with employers and teams.
• Facilitate community integration and life skills development with person-centered support and progress tracking.
