Job summary

Service Access & Management, Inc. (SAM) seeks a Service Coordinator for its Early Intervention service line in Berks County, PA.

You will provide case management to children 3 and under with developmental delays, working with families, caregivers, and professionals to secure needed services. The role involves coordination, advocacy, and documentation, with a start rate of $23.651/hour under the current union agreement.

Qualifications

• Bachelor’s degree with 12 college credits in EI or related social sciences and 1 year relevant experience.
• Associate’s degree with 60 credits in EI or related fields and 3 years relevant experience.
• Ability to work independently and manage a dynamic caseload.
• Strong interpersonal, organizational, and communication skills.
• A valid driver’s license and reliable transportation.

Responsibilities

• Request/complete service authorizations.
• Comply with OCDEL standards.
• Conduct Family Service Plan process with quality and compliance.
• Participate in Multi-Disciplinary Team to support outcomes.
• Complete referrals to agencies for child support and services.
• Facilitate Transition Plan to ensure continuity of care.
• Complete/update service plan as needed.
• Maintain positive relationships.
• Complete written documentation.
