Job summary

Great Lakes Bay Health Centers is seeking a Community-Based Doula/Perinatal CHW to support pregnant and postpartum families in Bay City, MI. You will visit homes, attend labor and delivery, and help families prepare for hospital care.

The role requires HS diploma, MDHHS doula certification (or to complete within 3–9 months), strong communication skills, and the ability to work flexible hours, some evenings and weekends. Travel within the service area is required.

Qualifications

• Education: HS diploma; MDHHS doula training or completion within 3 months.
• Licensure: MDHHS doula certification; Lactation Counselor preferred.
• Experience: Extensive childbirth experience with labor/delivery and postpartum support.
• Skills: Computer skills; problem solving; self-motivation; decisive; organized.
• Interpersonal Skills: Effective communication with diverse populations.
• Physical: Medium effort.
• Hours: Full-time/Part-time; some evenings/weekends.
• Travel: Reliable transportation, valid license, auto insurance; travel within service area.
• Education: CHW/CHR training or completion within 9 months; CPR/BLS certification.
• Experience: Care coordination/health promotion in recent years in health settings.
• Bilingual (English/Spanish) preferred.

Responsibilities

• Initiates contact with assigned families as early in pregnancy as possible.
• Attends scheduled appointments and maintains timely availability.
• Visits pregnant and parenting women at home or other settings per client preference.
• Uses birth plans, techniques, and materials to prepare for labor and parenting.
• Attends and supports the mother and family through labor and delivery and the early hours postpartum.
• Documents billable services in EMR per Michigan Medicaid Bulletins.
• Maintains postpartum contact to support breastfeeding and infant care up to 12 months.
• Develops relationships with community partners to ensure access to patients in hospitals and birthing teams.
• Maintains confidentiality and makes appropriate referrals.
