Job summary

D&S Community Services, a part of Sevita, seeks a Program Supervisor to lead direct support teams and oversee the implementation of Individual Service Plans (ISP). You will supervise Caregivers and coordinate daily activities, including transportation and community outings, to empower individuals we serve.

Ideal candidates bring at least 1 year of related experience, strong communication, and organizational skills.

Qualifications

• High School diploma or equivalent required.
• At least 1 year related work experience.
• Must be 18 years or older.
• Current driver’s license, car registration, and auto insurance may be required by regulatory authority.

Responsibilities

• Facilitate implementation of Individual Service Plans (ISP) and assist with staff hiring, training, scheduling, and oversight.
• Supervise a team of Caregivers supporting individuals in the program.
• Provide support to individuals including daily activities, meal preparation, medication administration, and personal care.
• Balance direct support with professional or program activities and supervision.
• Facilitate community involvement by accompanying individuals on outings or providing transportation to work or activities.
