Job summary

United Counseling Service of Bennington County Inc. in Bennington, Vermont seeks a Clinical Care Coordinator to support chronically mentally ill adults in the community under Outpatient Division guidance.

The role involves coordinating services, outreach, and ensuring access to health and social supports. You will travel to clients in the community, assist with income support and transportation, and participate in ongoing professional development, including clinical supervision for licensure and

Qualifications

• High school diploma and training in Motivational Interviewing and Collaborative Network Approach modalities.
• Preferred Bachelor’s degree in Social Work, Psychology or related Human Services field or equivalent experience (4+ years).
• Valid Driver’s License.
• Computer skills: typing and email usage.

Responsibilities

• Provides Clinical Care Coordination, supportive counseling and skill building for assigned clients in the community.
• May be assigned to provide Substance Use Disorder (SUD) services, or within related SUD grants.
• Provides outreach services to all in the community who are in need of services and helps with transportation as needed.
• Completes clinical and administrative forms for billing, information and records; maintains confidentiality at all times.
