Job summary

United Counseling Service of Bennington County Inc is seeking a compassionate Clinical Care Coordinator to deliver community-based mental health services. You will coordinate care, provide supportive counseling, and help clients access needed resources in their own environments.

Travel across Bennington County may be required to meet clients and coordinate services, with emphasis on HIPAA and 42 CFR Part 2 compliance and ethical professional standards.

Qualifications

• Minimum qualifications: high school diploma and training in Motivational Interviewing and Collaborative Network Approach modalities.
• Preferred qualifications: Bachelor's degree in Social Work, Psychology or related Human Services field, or equivalent experience of 4+ years.
• Valid Driver’s License.
• Computer skills: Typing and navigating email.

Responsibilities

• CONSUMER SUPPORT: Provide clinical care coordination, supportive counseling and skill-building for assigned clients in the community.
• May travel within Bennington County to meet clients and coordinate services.
• Assist with income support and entitlement; foster peer supports; support community and family involvement.
• Complete clinical and administrative forms for billing and records within required timelines.
