Qualifications

• High school diploma with training in Motional Interviewing and Collaborative Network Approach modalities.
• Preferred Bachelor’s degree in Social Work, Psychology, or related Human Services field, or equivalent/4+ years experience.
• Valid Driver’s License.
• Computer skills: Typing and emailing.

Responsibilities

• Provides Clinical Care Coordination, supportive counseling and skill building in a community-based setting.
• Travel to clients in Bennington County to provide services.
• Assist clients with income support and entitlements; foster peer supports.
• Maintain confidentiality and complete case management documentation promptly.
• Coordinate services and communicate care and planning with involved parties.
