Job summary

Health Connect America is seeking a Client Support Specialist for the Georgia HOPE program. This community-based role focuses on helping individuals and families build skills, access resources, and move toward greater independence. You’ll work in home and community settings, with supervision from therapists and teams, while maintaining documentation and coordinating care.

Qualifications

• High School Diploma with at least two years of relevant experience, or a Bachelor’s degree in a behavioral health or related field
• Experience working with children, adolescents, or adults with mental health or behavioral health needs preferred
• Valid driver’s license and reliable transportation for community-based work

Responsibilities

• Deliver community-based support services in homes and other real-world settings, meeting clients where they are
• Maintain regular contact with assigned clients, including in-person and phone-based support, to monitor needs and progress
• Review client history, assessments, and treatment plans to ensure services align with individualized goals
• Support clients and families in developing and achieving goals outlined in their IRP
• Teach coping strategies, emotional regulation, and daily living skills to promote independence and symptom management
• Collaborate closely with therapists, clinical teams, and community partners to ensure coordinated, whole-person care
• Track service utilization and maintain accurate, timely documentation in the EMR system
• Participate in team meetings, case staffing, and ongoing training to ensure high-quality service delivery

Education
