Job summary

Georgia HOPE, part of Health Connect America, is seeking a Client Support Specialist to provide community-based services. You’ll help individuals and families gain independence, build skills, and access resources in homes, schools, and local settings.

Responsibilities include direct client support, monitoring progress, coordinating with therapists, and documenting in the EMR. A driver’s license and reliable transportation are required; a degree in behavioral health is preferred.

Qualifications

• High School Diploma with at least two years of relevant experience, or a Bachelor's degree in a behavioral health or related field.
• Experience working with children, adolescents, or adults with mental health or behavioral health needs preferred.
• Valid driver’s license and reliable transportation for community-based work.

Responsibilities

• Deliver community-based support to clients in homes and real-world settings.
• Maintain regular contact with assigned clients via in-person and phone-based support.
• Review client history and treatment plans to ensure services align with goals.
• Support clients and families in developing and achieving goals outlined in their IRP.
• Teach coping strategies and daily living skills to promote independence.
• Collaborate with therapists and teams to ensure coordinated care.
• Document service utilization in the EMR system.

Education

Bachelor’s degree in behavioral health or related field
