Job summary

G&A Counseling in Richmond, KY is expanding our Targeted Case Management program and seeking dedicated case managers to join our team. You will work in the community connecting clients with medical, social, and educational resources.

This full-time role reports to the TCM Program Manager and offers opportunities for professional growth while coordinating with therapists, schools, and outside agencies to develop individualized care plans.

Qualifications

• Bachelor's degree in Social Work, Psychology, Human Services, or related field.
• Valid Kentucky driver's license and reliable transportation.
• Experience in community-based mental health preferred.
• Strong organizational and time management skills.
• Familiarity with Medicaid TCM billing preferred.
• Community-based work (not desk-bound).
• Clinical supervision and professional development.

Responsibilities

• Conduct community-based client visits.

• Develop and maintain individualized care plans.
