Paragon Behavioral Health Connections in Colorado seeks a Bachelor-Level Care Manager to deliver intensive community-based care management through the CAP Program. The role engages adults with SMI/SPMI, coordinating services across behavioral health, medical, housing, employment, and social systems in clients' homes and communities.

Qualifications

• Knowledge of Serious Mental Illness (SMI), Severe and Persistent Mental Illness (SPMI), and co-occurring disorders.
• Understanding of recovery-oriented, trauma-informed, person-centered care approaches.
• Knowledge of Medicaid, Social Security, housing resources, waiver programs, and community support systems.
• Strong crisis de-escalation, engagement, and rapport-building skills.
• Excellent written, verbal, and interpersonal communication skills.
• Strong organizational and time-management skills.
• Understanding of HIPAA, confidentiality requirements, and ethical service delivery.
• Commitment to equity, cultural humility, and serving diverse communities.

Responsibilities

• Provide intensive, community-based care management services in clients' homes and natural environments.
• Conduct outreach and engagement activities with individuals who experience barriers to treatment participation.
• Coordinate services across behavioral health, medical, housing, employment, vocational, and social service systems.
• Assist clients in identifying strengths, needs, goals, and barriers to recovery.
• Assist with obtaining identification documents, benefits, transportation resources, healthcare access, and community supports.
