Job summary

R&C Consulting Group, Inc. is seeking a Qualified Professional (QP) for the Community Support Team to provide community-based behavioral health support and care coordination.

The role collaborates with clients, families, and clinical professionals to support recovery, independence, and access to services. The QP implements service plans, monitors progress, documents outcomes, and participates in team meetings to ensure continuity of care.

Qualifications

• Degree/education in behavioral health, human services, psychology, social work, counseling, or related field as applicable to QP.
• Strong written and verbal communication abilities.
• Excellent organizational and documentation skills.
• Ability to work with clients in community and office settings.
• Knowledge of HIPAA and NC Medicaid requirements is preferred.

Responsibilities

• Provide community-based support and care coordination aligned with individualized service plans.
• Identify client goals, strengths, needs, and barriers to recovery.
• Coordinate access to behavioral health, housing, transportation, and employment supports.
• Assist with implementing service plans and monitoring progress toward goals.
• Document outcomes and communicate concerns to supervisors and clinical staff.
• Maintain confidentiality and adhere to professional boundaries.
