Qualifications
- Bachelor’s degree in Behavioral Health, Social Work, Counseling, Psychology, or related field.
- At least 2 years of direct experience with individuals diagnosed with Serious Mental Illness (SMI).
- Experience working on an ACT Team is preferred.
Responsibilities
- Provide community-based behavioral health services to individuals with Serious Mental Illness (SMI).
- Collaborate with a multidisciplinary ACT Team.
- Conduct ongoing assessments of member strengths and needs.
- Assist members in accessing community resources.
