Job summary

Ak-Chin Indian Community Health and Human Services is seeking a Case Manager to respond to targeted community needs and support its mission. Under the direct supervision of the HHS Director, the Case Manager will integrate best practices in social work and be flexible to work beyond normal hours when needed.

Qualifications

• Bachelor's Degree in Social Work, Public Health, Psychology, Education, or related field and three (3) years of professional level work experience in social work or an equivalent combination.
• Valid Arizona driver’s license and ability to meet insurance requirements.
• Must complete an extensive fingerprint/background check and be alcohol/drug free.
• Experience with Native American Communities or Tribes preferred.

Responsibilities

• Provides intake services for referrals for General Assistance, Burial Assistance, Homemaker Services, Emergency Assistance, Adult Protective Services and Adult Institutional Care and follows up with clients.
• Coordinates with other agencies to provide emergency assistance and resources.
• Coordinates, assigns and schedules Homemaker staff and trains new/current homemaker staff.
• Develops Individual Self-Sufficiency Plans for clients and maintains client files.
• Works with tribal, state, and federal programs to coordinate client services and maintain service integration.
