Job summary

MaineGeneral Medical Center’s Assertive Community Treatment (ACT) team provides comprehensive, community-based services to adults with severe mental illness. Staff meet clients where they live, work, and engage in daily activities to support recovery, housing stability, and improved quality of life.

Qualifications

• MHRT/C (Community) certification required.
• Valid driver's license and reliable transportation required.
• Experience working with adults with severe and persistent mental illness preferred.

Responsibilities

• Provide community-based case management to adults with severe mental illness.
• Meet clients in homes and in the community to assess needs and promote recovery.
• Collaborate with psychiatrists, nurses, social workers, counselors, peer specialists and others to develop treatment plans.
• Assist clients with healthcare access, housing, employment, benefits, transportation and community resources.
• Monitor progress, identify barriers, and advocate for needed services.
• Participate in crisis intervention, hospitalization prevention, discharge planning and transitions of care.
• Maintain accurate documentation per organizational, state, and federal requirements.
