Job summary

Elaya Health seeks a Nurse Practitioner or Physician Assistant to deliver on-site endocrine care to residents in skilled nursing and long-term care facilities around Columbus. You will manage diabetes, thyroid disease, and other endocrine conditions while partnering with facility nurses, primary care, and administration.

Qualifications

• Licensed NP or PA in Ohio or eligible for licensure.
• Experience in endocrinology, internal medicine, geriatrics or skilled nursing preferred.

• Patient-centered and focused on long-term care quality.

Responsibilities

• Perform on-site evaluations and follow-up visits.
• Manage diabetes, thyroid disorders, and other endocrine conditions.
• Review blood glucose trends, labs, and medication regimens.
• Optimize diabetes management and reduce finger sticks and medication burden.
• Collaborate with nursing staff, facility leadership, and primary care teams.
• Provide clear clinical recommendations and ongoing follow-up.
• Document in facility EMRs (training and administrative support provided).
• Help reduce avoidable hospitalizations through proactive specialty care.

Compensation: $150,000 – $220,000 annually

Elaya Health is a rapidly growing endocrinology practice specializing in high-quality care for patients in skilled nursing and long-term care facilities. We are seeking an experienced, compassionate Nurse Practitioner or Physician Assistant to join our expanding team.

About the Role

Our providers deliver on-site endocrine care to residents of skilled nursing and long-term care facilities throughout the Columbus area. You will manage patients with diabetes, thyroid disease, and other common endocrine conditions while collaborating closely with facility nursing teams, primary care providers, and administration.

What You’ll Do

• Perform on-site patient evaluations and follow-up visits
• Manage diabetes, thyroid disorders, and other common endocrine conditions

Qualifications

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in Ohio, or willingness to obtain licensure
• Experience in endocrinology, internal medicine, geriatrics, or skilled nursing preferred
• Endocrinology experience is a plus — not required for the right candidate
• Strong clinical judgment and communication skills
